Financial Policy
Payment in full is expected for all consultation visits. We will file a claim to your insurance company whenever possible as we are not contracted providers for any insurance company. Please note that some insurance companies will not allow us to file claims on behalf of the patient.

We accept cash, money orders, personal checks, Visa, Mastercard, Discover, American Express and CareCredit.

We strive to help every patient obtain the maximum allowable insurance benefit for the services that we provide. As a courtesy to our patients, our insurance coordinators will attempt to estimate your benefits in advance of your surgery whenever time permits (sometimes this is not possible with same or next-day surgery). In order for us to do this, you will need to bring in your insurance ID card for both medical and dental coverage. If we are able to verify your eligibility, deductibles, annual maximum, excluded services, and coverage for your particular surgical procedure(s), we will accept your estimated portion of the fee at your surgery visit, and file the claim with your insurance company. Please understand that this is an estimate only, and not an absolute guarantee of any coverage. The insurance company ultimately makes the final decision regarding coverage. If any of this information is unavailable to us, or your insurance company's policies do not allow for direct payment to the healthcare provider(s), we will collect the entire fee at your surgery appointment and promptly file a claim with your insurance company directing any reimbursement be paid directly to you. If you prefer, we can provide you an itemized receipt so you can file with your insurance company directly. We do everything possible to make this process as convenient as possible for our patients.

Our office also accepts CareCredit. Many patients put off treatments and procedures because they cannot afford the costs of treatment. We offer CareCredit payment plans as a convenient alternative to consumer credit cards, cash or checks. CareCredit allows you to finance the costs of treatment over a pre-determined time period, as opposed to paying the full cost of treatment up front. CareCredit allows our patients to enjoy the benefits of treatment while they make payments that fit their budget. We offer a 6 month deferred interest period. Please note that Doctors Braxton and Chicola are not Medicare or Medicaid providers, and as such, are not allowed to file Medicare or Medicaid claims on behalf of the patient. Patients will not be allowed to file a claim on their own either.
